Position Summary

Under the leadership of the Diabetes Services Program Manager, the Diabetes Services Educator position is responsible for providing patient education to patients in the inpatient, outpatient and community settings. The Educator is an active member of the health care team and participates in multi-disciplinary rounds. The Educator must be able to interact empathetically and compassionately with a variety of patients and their families concerning chronic, acute and critical care medical needs. The Educator must be able to articulate the medical science related to diabetes and translate their knowledge into layman's terms in one-to-one counseling sessions, group classes, public speaking engagements, while working with print, audio, and video media. The Diabetes Educator possesses a broad scope of knowledge and skills to provide person-centered education to facilitate patient self-care management.

Experience 

Must have at least 3 years of broad clinical nursing experience. A focus in diabetes education is highly preferred. Fluency, the ability to communicate orally, and the ability to read and write in English is required. The ability to project a positive, warm, friendly and healing attitude to the patients of Community Hospital and to the community is required. Excellent customer service is required. Two years experience as a Diabetes Educator working with patient education is preferred. Formal and informal teaching experience to include facilitating and teaching individuals and groups is preferred. Ability to effectively communicate in Spanish and additional foreign languages is strongly preferred.

Education

Bachelor’s degree in nursing strongly preferred.

Must pass orientation and initial competency assessment prior to independent assignment of patients requiring the applicable skill, and must complete and pass all annual competencies.

Licensure/Certifications

State of California RN license required. Certified Diabetes Care and Education Specialist (CDCES) certification preferred upon hire but required within 18 months, or at the completion of 1000 hours of providing patient diabetes education. American Heart Association Healthcare Provider BLS certification required.

Assigned Work Hours:

Full Time

Variable shifts: 0730-1600 and/or 0800-1630.

Intermittent weekend shifts.

Position Type:

Pay Range (based on years of applicable experience):

$67.29

to

$89.98

The hours employees work determine when a shift differential is paid.

Hourly Evening Shift Differential: $5.50

Hourly Night Shift Differential: $8.00
