Integrative HIT facilitates connectivity across the care-site continuum, the development of evidence-based clinical decision support tools, and performance measurement systems. It also supports patient engagement by providing information to empower patients to be active participants in fostering and maintaining their own health.

In addition, CHI engages payers in designing value-based insurance products that promote evidence-based workplace wellness and primary prevention and disease management to keep our community healthy and reduce the clinical and financial burden of disease.

Purpose of Position
The priority goal for a Patient Navigator is to connect Central California Alliance for Health (CCAH) Medi-Cal members to their healthcare homes where they can receive appropriate levels of care with the desired outcome being improved health for designated patient populations.

This position will be located in the hospital Emergency Department and will work directly with CCAH members to:
* Schedule appointments and coordinate the referral process between hospital emergency departments (EDs) community health centers/primary care clinics, and social supportive resources.
* Transfer relevant patient health data via a secure web-based system, as well as necessary follow-up to ensure continuity of care
* Provide patient education and ensure patient is knowledgeable about resources available to them through their CCAH membership

Position Requirements
* Bilingual - English/Spanish
* High School diploma or GED required. Completion of an Associate Arts degree is preferred with an emphasis in social services, psychology or any health related field.
* At least two years' experience in a health care, social service setting including any experience conducting community outreach, case management, interpreting or patient-focused services.
* Must be able to work flexible hours (morning, afternoon, and /or evening shifts including weekends as needed. The hours will be 10:00 am - 6:30 pm
* Strong interpersonal skills, with a demonstrated ability to work independently on individual and assigned tasks while exhibiting good judgement.
* Provide information in a manner that is culturally and linguistically appropriate to the needs of each patient.
* Ability to work independently in a fast-paced demanding emergency department environment and coordinate several tasks simultaneously.
* Ability to maintain professionalism with hospital clinicians and professionals as well as other community organizations and maintain strict confidentiality.
* Serve as a resource for patient, other navigators and hospital staff.
* Effective written and oral communication skills.
* Working knowledge of Microsoft Office applications.
* Problem solving, decision making and critical thinking skills.
* Excellent organizational skills with the ability to prioritize assignments.
* Utilize good judgement and handle confidential and sensitive issues with tact and diplomacy and to work with culturally diverse populations.

Preferences
* Strong knowledge of community resources in Monterey County.
* Familiarity and working knowledge of Medi-Cal and other public programs.
