The patient-centric team approach is supported through in-office and community-based clinical services, and integrative health information technology (HIT). Integrative HIT facilitates connectivity across the care site continuum and enables the development of evidence-based clinical decision support tools, and performance measurement systems. They also support patient engagement providing information to empower them to be active participants in fostering and maintaining their own health. In addition, CHI engages payers in designing value-based insurance products that promote evidence-based workplace wellness, primary prevention and disease management to keep our community healthy and reduce the clinical and financial burden of disease.

Purpose of Position

The CHI Data Specialist is responsible for supporting the integrated Population Health goals of Community Health Innovations (CHI). This position assists with data file coordination, loading, cleanup and investigating discrepancies and errors through integrated CHI systems. This position requires the capability to aggregate various data files, manipulate data in Excel and SQL, and create robust reports for executive leadership. Relies on experience and judgment to plan and accomplish goals. A degree of creativity and latitude is required.


  1. Identifies, coordinates and aggregates data from a variety of sources.
  2. Collaborate with clinical and operational teams in the design and development of executive management level reports.
  3. Responsible for data testing, file load coordination and validation which requires a highly detail oriented individual.
  4. Using Excel and SQL, researches, identifies and proposes solutions for data discrepancies.
  5. Other duties as assigned.

Position Requirements

  1. Bachelor's degree or equivalent education/experience in business administration, medical practice management, informatics, computer science or a similar discipline.
  2. 3-4 years' minimum of relevant experience in the healthcare industry.
  3. Previous electronic health record or other healthcare system technical experience is required.
  4. Professional demeanor with effective, tactful, and persuasive written, verbal, and nonverbal communication skills.
  5. Previous client and/or patient customer service experience is preferred.
  6. Extremely strong computer skills in addition to strong capabilities in MS Word, Excel, Project, PowerPoint, and Visio.
  7. Strong SQL abilities.
  8. Ability to balance multi-stakeholder goals and objectives.


  1. Experience with Essette (Population Management Software system)
