The patient-centric team approach is supported through in-office and community-based clinical services, and integrative health information technology (HIT).

Integrative HIT facilitates connectivity across the care site continuum and enables the development of evidence-based clinical decision support tools, and performance measurement systems. They also support patient engagement providing information to empower them to be active participants in fostering and maintaining their own health.

In addition, CHI engages payers in designing value-based insurance products that promote evidence-based workplace wellness, primary prevention and disease management to keep our community healthy and reduce the clinical and financial burden of disease.

Purpose of Position
The Pediatric Wellness Coach role was created to support the goals of the Monterey Salinas Healthcare Collaborative (MSHC) as we continue to implement the Diabetes Initiative. MSHC is an initiative that grew from the commitment to work together in new and meaningful ways, of leaders at Salinas Valley Memorial Hospital (SVMH), Community Hospital of the Monterey Peninsula (CHOMP) and the Monterey Bay Independent Physician Association (MBIPA). The purpose of the initiative is to prevent the development of diabetes and other obesity related diseases, as well as provide positive, measureable change for those already diagnosed with diabetes and other obesity related diseases among our Monterey County residents.

The Pediatric Wellness Coach will be an employee of Community Health Innovations (CHI) working directly with the Diabetes Initiative Coordinator (DIC). The Pediatric Wellness Coach will be embedded within multiple different Pediatrician offices' and several other community locations serving our county's pediatric population. The Pediatric Wellness Coach will implement the initiatives goals to provide community based strategies emphasizing disease prevention within our pediatric population.

Responsibilities:

The Pediatric Wellness Coach will work closely with pediatric clients and family members in promoting wellness through 1:1 visits and group education, helping families to achieve optimal health through lifestyle modification, establishing goals and action plans, and conducting follow-up to support healthy sustainable behavior change.

  1. Provide health coaching activities across the continuum of care in order to facilitate and promote high quality, cost effective outcomes, focused on the whole client and family orientation and self management decision support.
  2. Act as a liaison between the client, the client's support network, treating physician(s), ancillary providers and community resources utilizing a multi-disciplinary approach following protocols to assess, identify, plan, goal setting and measure optimal clinical outcomes.
  3. Employ behavior change and motivational interviewing skills to assess readiness, health goal setting, short and/or long term needs, engage member's plans for change following standard policy and procedures, clinical guidelines and national evidenced-based criteria.
  4. Work collaboratively with Diabetes Initiative Coordinator, CHI team members and other stakeholders including but not limited to; physicians and their office staff, community members and resources to identify individuals who require additional disease management, wellness, and prevention services.
  5. Support the development, implementation and evaluation of the Pediatric Wellness Coach programs and outreach activities to improve optimal health outcomes. May include, but not limited to; program promotion, biometric screenings, managing wellness challenges, health education content development, providing health education lectures/class and health campaigns.
  6. Act as a patient advocate responding to and working to resolve client concerns or barriers.
  7. Perform additional duties as assigned.

Position Requirements:

  1. Associate of Science, equivalent or higher, in health related field (i.e. health, wellness, nutrition, exercise science, etc.) from an accredited college or university.
  2. At least one year experience as a health coach or providing impactful health promotion/disease management services (clinical setting preferred).
  3. Bilingual English/Spanish fluency.
  4. BLS certification prior to employment.
  5. Valid driver's license and personal transportation.
  6. Excellent oral and written communication skills.
  7. Demonstrate cultural competence.
  8. Apply motivational interviewing/behavioral change techniques.
  9. Tailor information at the appropriate level of literacy.
  10. Ability to work as an integral member of an interdisciplinary team.
  11. Strong customer service skills with ability to effectively establish rapport with clients in person and via telephone.
  12. Ability to develop and implement wellness challenges and engagement activities including health education materials.

Preferences:

  1. Certified Health Coach.
  2. Experience working with pediatric population.
  3. Experience facilitating group education.
  4. Experience developing multimedia messaging and advertisement.
